Privacy Policy

Canopy Pines Recovery Center is committed to protecting the privacy and confidentiality of our patients’ information in accordance with the Health Insurance Portability and Accountability Act (HIPAA), 42 CFR Part 2 (federal confidentiality regulations for substance use disorder records), and applicable Florida law.

Protected Health Information (PHI): We collect and maintain PHI, including personal, medical, and treatment information, solely for the purposes of providing care, coordinating treatment, processing insurance and billing, and complying with legal and regulatory requirements. We do not sell patient information.

Medical Records: Patient medical records are maintained securely and confidentially. Access is limited to authorized clinical and administrative staff directly involved in a patient’s care. Records are retained in accordance with Florida Department of Children and Families (DCF) licensure requirements and applicable federal law.

Confidentiality of Substance Use Disorder Records: In accordance with 42 CFR Part 2, records related to substance use disorder treatment are subject to additional federal confidentiality protections and generally may not be disclosed without the patient’s written consent, except as permitted by law.

Patient Rights: Patients have the right to:

  • Access and request copies of their medical records
  • Request corrections to their records
  • Receive an accounting of disclosures of their PHI
  • Request restrictions on certain uses or disclosures
  • File a complaint if they believe their privacy rights have been violated

Disclosure of Information: We do not disclose PHI to third parties without patient authorization, except as required for treatment, payment, healthcare operations, or as otherwise permitted or required by law.
